The Town of Dover is accepting contact information from residents who want to receive a “Yellow Dot” health information record kit courtesy of the NYS Sheriff’s Association.
To order a kit, please call the Town Clerk's Office at (845) 832-6111, then option 2 or email Katie Palmer-House at townclerk@doverny.us.
The kit comes with two health information sheets (complete and put one in your car’s glove compartment and one in a plastic bag in your freezer).
Put a decal on your driver’s side passenger window and a decal on your front door window.
